Job Description
FAC Case Manager
Midwest Asian Health Association (MAHA) is a non-profit organization with a mission to reduce health disparities for Asian and Pacific Islander Americans and other low-income populations through culturally sensitive and linguistically appropriate community health programs.
Summary of Roles and Responsibilities:
The FAC Case Manager will support families and children in MAHA’s Family Advocacy Center (FAC) by helping parents pursue and achieve goals that promote family preservation and reunification. This position will be stationed in the Uptown neighborhood of Chicago.
Duties and Responsibilities:
- Conduct intake, assessments, home visits required by DCFS together with MAHA's Family Advocacy Center project coordinator, or other staff.
- Develop service plan for the participants, including safety planning and the plan for youth development and children's wellness.
- Assess and identify referral needs of the clients and develop a referral plan, link the clients to the support services, and provide proper follow-ups.
- Provide case management, counseling, and support services to children, youth, adults, and families, especially survivors of domestic violence and sexual assault as they engage with the legal services.
- Communicate with DCFS regarding the clients and their pending cases and assist them to achieve family unification and stable/permanent homes for children and clients.
- Maintain/update case notes for each client as required by the DCFS in a timely manner.
- Responsible for data collection, data entry, and program evaluation.
- Work closely with MAHA's Community Mental Health Clinic team to maintain counseling and mental health support for participants experiencing trauma and receive psycho-social assessments, crisis intervention, and follow up services.
- Collaborate with the project coordinator and community resource coordinator to identify referral and supportive services for the clients.
- Attend DCFS meetings and MAHA FAC team meetings on a regular basis.
- Attend required trainings.
- Other duties as assigned.
Experience and Qualifications:
- Bachelor’s degree in social work, counseling, or a related field. Master’s degree preferred.
- Strong interpersonal and communication skills, with the ability to engage diverse populations.
- Excellent communication skills, fluent in Chinese and/or Vietnamese preferred.
- Experience in case management.
- Computer proficiency with Microsoft Office
- Able to drive with a valid Illinois driver license and auto insurance
Salary and Benefits
This is a full-time position following a three-month probationary period. The salary range shall be $40,000 to $50,000, commensurate with experience, qualifications, and available funding.
