Updated on March 27, 2017 by MAHA Navigator Program

Although the 2017 Open Enrollment Period for the Health Insurance Market-place has ended, you may be eligible for a Special Enrollment Period (SEP) if you have experienced a life change within the past 60 days or expect to in the next 60 days. These life changes include:

  • Loss of health insurance (such as job or school-based coverage or Medicaid)
  • Changes in household size (due to marriage, divorce, birth of a child, etc.)
  • Changed in residence (such as moving to a new ZIP code or from a foreign country)
  • Changes in immigration status (Permanent Resident, US Citizen, Employ-ment Authorization, etc.)

To see if you qualify and apply, visit HealthCare.gov or call 1-800-318-2596.

You can also contact us for any health insurance related questions by calling 312-225-8659/312-225-6806.

This program is made possible by a grant from Health Human Services (HHS) through United Way.
